Due to a change in tax law, the IRS does not require submission of your Form 1095-B to file your federal income taxes. Most insured members will no longer automatically receive a paper copy.

To receive a copy of your Form 1095-B, members can send a request including your name and ID# to Elite Administration using one of the following methods:

Upon receiving the request and your permission, Form 1095-B will be sent to you via email no later than 30 days following your request.
